Telehealth may be covered.

If insurance declines to cover visit(s) for any reason, you are responsible for the full amount. Upon request, we can also provide you with a Superbill to submit to insurance yourself for possible reimbursement. Payment is due from you at the time of service. HSA/FSA may also be used for payment.

To avoid financial surprises, before your visit, it is recommended you call your insurance company
& ask the following questions & report back to me (& write down the date & reference number for the call):  
-Is nutrition counseling covered? (procedure codes 97802/97803)
-Are there restrictions on which diagnoses are covered?
Is Z71.3 (dietary counseling and surveillance) covered, or is a medical diagnosis required?
-Do I need a referral or prior authorization?
-Is nutrition counseling covered when provided via telehealth?
-How many visits are allowed?
-Do I have a deductible to meet before insurance will pay? If so, can the deductible be waived for preventive nutrition therapy?
-Will I have a copay or coinsurance?
-Is nutrition covered under my plan? (they may ask for provider NPI, contact me for that)
